What you should bring to your appointment:
- Insurance card and Photo ID (License/State ID, passport, Military ID)
- We request a copy of the front of your insurance and ID for your file; you may take a picture/screenshot and email to office@elitefootandanklecenter.com, or we can scan it in the office during your visit.
- Medication List if applicable
- X-rays, MRIs/other imaging on a disc, if applicable
- Method of payment for copay if applicable (Cash, Check, Visa/Mastercard, Amex, HSA/FSA). Your copay amount is dictated by your insurance plan and is due at the time of check-in.
- We have a QR code in-office to pay your copay online, or we can manually run your card if preferred.
New Patient Forms
We have made an effort to minimize required paperwork for your visit. Our new patient paperwork is 3 pages consisting of Demographic information, Financial Policy, and HIPAA form.
These forms can be pre-printed and completed prior to your appointment, please email completed paperwork to office@elitefootandanklecenter.com. You may also fill out the forms in-office, please allow about 5-10 minutes before your scheduled appointment time.
